MADISON HIGH SCHOOL-BASED HEALTH CENTER

AVAILABLE FOR SERVICE: AUGUST THROUGH MAY WHEN CHILDREN ATTEND SCHOOL FROM 8:00 A.M. UNTIL 4:00 P.M.

 

SERVICES OFFERED:  Medical Health: Child care including well and sick visits, immunizations, sports physicals, acute and diagnostic care. 

Behavioral Health: Individual and group therapy including stress and anger management, dealing with anxiety, bullying, substance abuse, grief counseling and communication.

Medical and Mental Telehealth:  Some medical and behavioral services may be provided via telehealth.  Telehealth covers ways you and your provider can use phone calls, video chats, emails, and text messages.  All your child needs to use telehealth is reliable internet and a phone, smartphone, or computer.

INFORMATION ABOUT MADISON HIGH SCHOOL-BASED HEALTH CENTER:

Telehealth services are now being provided as discussed under “Services Offered.”  Parents will sign separate forms for these services.  We ask that you sign the Telehealth Medical and Telehealth Mental Health forms for your child.  Both services are oftentimes provided at some point and time during the school year.

INFORMATION ABOUT MADISON HIGH SCHOOL-BASED HEALTH CENTER:

Madison High School-Based Health Center is located on the campus of Madison High and Madison Middle Schools.  It is the building standing alone that is located straight-ahead when you come through the gate of the campus.

 

Students can be seen at the school-based health center with parental permission.  This permission is given through the completion of a Consent Form.  This Consent Form remains in effect while your child is enrolled in school, but we do send a short Consent Form Update annually to be completed.  This lets us know if there are pertinent changes we should know about.  These forms can be downloaded from the school website.  However, we do send the appropriate form home with your child shortly after the start of school.

All enrolled students are provided care by Madison High School-Based Health Center at no out of pocket cost regardless of insurance status.  Insurances are accepted and billed.

 

Services are available to all school students, but I am sorry we are unable to provide transportation.  However, parents may transport their children who are not on the campus to Madison High School-Based Health Center for services.  Also, please be aware that a school nurse is available to students.  You will find information about the school nurse on this web page.  Access the school nurse pages for further information.
